Passport & Birth

Passport book applications are accepted at the Acadia Parish Clerk of Court office on the second floor.  Passport books are good for air, sea, or land travel.  In order to obtain a passport book, the applicant must have the following:

Proof of U.S. Citizenship, I.E.; a certified  copy of your birth certificate.

bullet

Proof of identity, I.E.; a valid driver's license and a copy front and back.

bullet

One (1) passport photograph (studio picture, 2"x2").

bullet

Fees are $110.00 for adults, age 16 and over (an adult passport is normally valid 10 years from the date of issue).

bullet

Fees are $80.00 for children, age 15 and under (a child passport is normally valid 5 years from date of issue).

Payable in check or money order to: U.S. Department of State
A $25.00 application fee, payable by separate check, money order, or cash is due the Clerk of Court.

Passport books generally take 6-8 weeks to process.  There is an expedite process for an additional $60 plus overnight return shipping for $12.72.  With this process, passports will be turned around in 10 working days with overnight mail from the post office.

If you need to renew a passport book that has not expired, you may pick up a renewal application in our office.  If your passport has expired, you will need to fill out a new application form and submit your expired passport.  If you have lost your expired passport, you may still fill out an application for a new one.

Children under the age of 16 require the signature of both parents or documentation as to why that is not possible.

If you are submitting an out-of-state identification, you must provide two (2) photo identifications.  Example, Driver's license, work I.D., government issued I.D, etc.

Birth Certificate and Birth Card applications are accepted at the Acadia Parish Clerk of Court office on the second floor.  In order to obtain, the applicant must have the following:

The requirements are:

  1. You must fill out a completed application which you can obtain from the Clerk's office

  2. You must present identification, which consists of one of the following:

Primary Documents
a. A state issued driver's license
b. A state issued picture identification.
c. A current U.S. Military identification card which includes a photograph
d. A high school year book provided the picture clearly identifies the applicant as the same.

Secondary Documents
a. A student I.D. Card for a Louisiana college or university accompanied by a 100% fee paid receipt for the current semester.
b. A W-2 form issued within the last two years plus a social security card.  The social security numbers must match.
c. Original documents of adoption papers.
d. Original Louisiana diploma from a high school or college.
e. Official certified deeds or title to property in Louisiana.
f. Louisiana vehicle registration or certificate of title.
g. Local utility statements showing name and address of individual applying (each utility company counts as one document).
h. Insurance policy (health, home, life, auto).
i. A military dependent I.D. card or credit card statement showing name of applicant and home address.  (Note: Credit cards are not acceptable.)

People who are eligible to get birth certificates with appropriate I.D. is as follows:
a. Registrant/Person named in the certificate
b. Mother/Father of the person named in the certificate
c. Children of the person named in the certificate
d. Current legal spouse of the person named in the certificate
e. Brother/Sister of the person named in the certificate
f. Grandparents/Grandchildren of the person named in the certificate
g. Legal guardian of the person named in the certificate.  Must have judgment stating such.
h. Non relative with Authorization from the person named in the certificate.  Must have a letter of authorization and a copy of applicant's state i.d. or driver's license.  Letter must state the registrant's name, registrant's date of birth, and the name of the person authorized to pick up the birth certificate.

You may  be required to fill out an affidavit stating who you are.

The birth certificates can be issued from 1983 until current and the birth cards can be issued going back 100 years.  For new borns, you have to wait ninety (90) days before you can obtain a birth certificate.

The fees for these services are:

Birth Certificates: $24.00
Birth Cards: $14.00

We can only accept checks, cash, or money orders for this service.  The check or money order should be made payable to the "Acadia Parish Clerk of Court".

Anyone may purchase a birth certificate or birth card provided that the proper application has been filled out.

A birth card can be used for anything that a birth certificate can be used for.
